Respite / Short stay  Accommodation

We offer short term accommodation that is fully equipped with state of the art gadgets to keep you company. Our respite accommodation is open 24-7 manned with qualified staff to assist you during your stay. You can stay as little for few hours to several weeks, depending on the amount of time you choose to live in with maximum comfort. This accommodation is designed to offer maximum convenient experience for our customers.

Community Participation

Social interaction and community engagement are essential parts of daily life. That’s why we believe in supporting you to connect and participate in your own community each day. We work closely with you and your support networks to identify community-based activities you are most interested in doing, goals you would like to achieve and the best way to achieve them. From there, we design community participation programs that support you to reach those goals. Our priority is to ensure every activity aligns with your interests and is suitable for your current social and communication skills. These activities take place in the community and may be: Within a group One-to-one with Grace & Mercy Care Services support worker. Our experienced staff are highly skilled in working with a diverse range of disabilities, including those with complex needs and are able to work with you and your loved ones to ensure all outings and activities meet your goals and needs.

Psychosocial Recovery Coach

We also offer recovery Our Psychosocial Recovery Coaches helps NDIS participant to make sense of their plan and utilise their funding. Recovery Coaches connect participants to the services they require (NDIS and community supports), coach participants to achieve their goals, and prepare for and resolve crisis if it arises. Additionally, coaches work closely with support workers to ensure that they understand how to best support participants in achieving their goals.

Supported Independent Living

Grace & Mercy Care Services offers different ranges of Supported Independent Living (SIL). participant has the choice of choosing any room in our housing units. This accommodation is shared between 2-4 people co-habiting together and sharing communal areas. The accommodation is also tailored to suit the needs of the participants residing in the accommodation. The accommodation is always staffed with 1-2 staffs on 24hours basis to give support and assistance in cooking, appointments, shopping, etc. and teaching daily life skills to facilitate recovery into full independent living.

Support Coordination

We offer support coordination across western Australia. Our qualified and experienced Support Coordinators are familiar with the local services and can offer you the information you need to make the best choice. Provide written reports, as required, on outcomes, successes, and barriers to support participant funding reviews. Strength and enhance the participant’s abilities for greater independence to coordinator supports and participate in the community in the longer term. If your NDIS plan includes support coordination, we would like to discuss your objectives and what you expect from a Support Coordinator.

Recovery Accommodation

We also offer recovery accommodation design to assist people living with mental illnesses to recover while they are awaiting transition into the community. This accommodation will assist those that are recently discharged from hospital to be equipped with daily life skills and build on their physical and mental capacity before reestablishing into the community. This accommodation is 24-7 fully staffed with qualified staff.
